Royal Flying Doctor Service of Australia Central Operations ACN 141 354 734 (RFDS SA/NT) is collecting this information from you for the purposes of receiving and then addressing feedback and as described here.

We may have collected information about you from another person who has provided feedback to us.  Otherwise we will collect information from you directly.  We may also receive information about you from other service providers or the public record.

We collect the information in this form to receive feedback about our services or experiences of people with RFDS SA/NT, and to update you about how we are dealing with that feedback if applicable.  We may use it for quality improvement and education purposes.  If you do not complete all of the information in this form or provide accurate information, we may not be able to adequately address any issues raised or make improvements, or give you updates about how we have addressed the feedback.

We also collect information so we can contact you about other services we offer, events, to deal with complaints or stakeholder management, and to comply with laws.

We may need to disclose your personal or sensitive health information if required by law, in the case of emergencies to other health service providers, and to third parties where you have consented for us to do so (either by providing that consent directly to us, or by requesting that third party to seek the information from us).
